Donation Request
To assist the team at Fazio’s determine how best to invest our charitable donations, we need to learn a bit more about your organization and the goals you are trying to achieve. Please complete the attached form below and email it with subject "Donation Request Form" to: Sweets@fazioschocolate.com. We will ONLY accept online donation requests.
DONATION REQUEST INFORMATION
For your cause to be considered for a donation, you must meet the following criteria:
- You must be a U.S. based IRS501(c)(3) qualified charitable organization.
- You will agree to pick the donation up at Fazio’s Chocolate Shop, located at 13425 Watertown Plank Rd, Ste #7, Elm Grove, WI 53122
We will not consider any of the following requests:
- Contributions to a political candidate or organization.
- Contributions to any organization outside of the State of Wisconsin.
- Contributions to organizations that discriminate by race, creed, gender, ethnicity, sexual orientation, disability, age, or any other legal basis prohibited by law.
- We will not consider donation requests by phone or verbal solicitations.
- Completing a Donation Request Form does not constitute a donation!

ATTENTION: Due to the large number of donation requests we receive, our policy is to review all emailed requests on a QUARTERLY BASIS during the last week of each quarter based on the calendar year. Please consider this when submitting your request.
